For all death claims we’ll need to see the original death certificate.

In addition, depending on circumstances, we may also request copies or evidence of any of the following:

  • Death claim form
  • Will
  • Marriage certificate
  • Civil Partnership certificate
  • Proof of ID for spouse
  • Proof of ID for Civil Partnership
  • Proof of ID for any other claimant
  • Grant of Probate
  • Evidence of address
  • Evidence of living at the same address as deceased
  • Evidence of relationship to deceased
  • Confirmation of length of relationship.
